Oh, I can think of lots of perverse ways to get this wrong. Let's see...
Step 1. Failed to specify that a counter is a work surface and not a counting aid, sandwich will be assembled on top of an abacus. Type of spoon not specified, assuming slotted serving spoon. "Dinner plate" does not specify what kind of dinner should be on the plate, assuming a premade PB&J sandwich for maximum irony. Did not specify that the peanut butter and jelly should be in separate jars, will mix them ahead of time - this doesn't actually hurt the recipe much, but demonstrates the point.
I can probably wring a few more out of step 1, but those are the ones that occurred to me on my first read through.
